Client Track

The Client Track database was developed by the National Gang Center to assist program staff with collecting program referral information, individual client data, and program services data. Program staff can use Client Track to document services and outcomes of clients who are being case-managed by a multidisciplinary intervention team. The data to be entered relate to treatment and services and cannot be used to identify and track gang members.

Client Track allows program staff to:

  • Track referrals made to the program
  • Document baseline data and ongoing information about program clients
  • Document case-management services
  • Assess individual client progress
  • Assess overall program outcomes

You can request permission to download the database or request a copy on CD through the online request form.

Client Referral Information includes:

  • Demographic data
  • Gang involvement
  • Status in legal system
  • School enrollment status
  • Reasons for referral
  • Referring agency information
  • Contact notes
  • Referral results

Enrolled Client Information includes:

  • Demographic data
  • Living situation, siblings, and dependents
  • Educational status
  • Employment status
  • Gang involvement
  • Criminal involvement
  • Substance use

Tracking information includes intake and update information for all clients enrolled in the program. This information is tracked over time and used to generate program management and outcome reports.

Service Information Reports include:

  • Client intervention plans
  • Service referrals and results
  • Client contact notes

System Reports include:

  • Initial referral management and outcomes
  • Individual client outcomes
  • Service dosage
  • Program outcomes summary
